Chinese version of the medication adherence scale for patients with hypertension:a systematic review of psychometric properties

Objective To evaluate the quality of psychometric properties and methodological quality of the Chinese version of the medication adherence scale for patients with hypertension,so as to provide references for the selection of high-quality research tools. Methods PubMed,EmBase and Web of Science,CNKI,Wanfang Data knowledge service,VIP database,Chinese Biomedical Literature database were retrieved to collect the literature on the evaluation of psychometric properties of medication adherence scales for patients with hypertension from the time of database establishment to February 3,2022. There were 2 reviewers who independently screened the literature,and extracted data of included studies. Based on the consensus-based standards for the selection of health measurement instruments(COSMIN) guideline,the methodological and measurement properties quality of the included assessment tools were evaluated by the COSMIN risk of bias checklist and quality criteria. Results A total of 10 studies were included,involving 9 Chinese version of the medication adherence scales for patients with hypertension. None of the 9 assessment tools reported cross-cultural validity,measurement error,hypothesis testing or responsiveness. As the content validity were inadequate or uncertain,and the quality of evidence were moderate or low,9 assessment tools were recommended by grade B. Conclusion Compared with the other 8 assessment tools,MASES-R is the assessment tool with the relatively balanced psychometric properties with good reliability and validity,which can be temporarily recommended for use,but it needs further testing.

Construction and application of a volume management program in vulnerable phase for patients with heart failure

Objective To construct a volume management program based on Information-Knowledge-Attitude-Practice(IKAP) theory and to discuss its application effect for patients with heart failure in vulnerable phase. Methods The 4 process and capacity management architecture based on IKAP theory were constructed by the expert group discussion method. Using the convenience sampling method,94 patients with heart failure from May to October 2021 were selected as the experimental group,and the same number of patients from May to October 2020 were selected as the control group,both of whom were admitted to a tertiary first-class hospital in the city of Nanchang. The experimental group received the volume management program based on IKAP theory,and the control group received the routine care of the department. Finally,we evaluated the effect of the intervention. Results 94 patients in the experimental group and 92 patients in the control group were finally collected. There were statistically significant differences in self-care ability,weight management ability,B-type natriuretic peptide level,dry body mass compliance rate and readmission rate between 2 groups after the intervention(P<0.05). Conclusion The volume management program based on IKAP theory can improve self-care ability,weight management ability and dry body mass compliance rate of vulnerable patients with heart failure,and reduce B-type natriuretic peptide level and readmission rate in vulnerable phase.

Influence of blood pressure fluctuation on operative side palm swelling in patients with radial artery compression after percutaneous coronary intervention

Objective To investigate the relationship between the changes of blood pressure and palmar swelling during radial artery compression in patients with radial artery percutaneous coronary intervention (PCI). Methods The patients who underwent radial artery PCI in the coronary heart disease unit of a tertiary general hospital in Nanchang from May 2018 to December 2020 were selected as the study subjects. The general data and the related indexes of decompression were analyzed retrospectively,and the blood pressure fluctuation during radial artery compression was described. Univariate regression analysis was used to analyze the influencing factors of palmar swelling on the operative side,and the blood pressure during radial artery compression was divided into nodes. The effects of different blood pressure on operative side palm swelling were analyzed by stratification,and the corresponding range of blood pressure at the highest risk of operation side palm swelling was further analyzed by trend regression. Results A total of 788 patients with radial artery PCI were included. Based on the analysis of patients’ baseline data,it was found that history of diabetes,history of hypertension,signs of heart failure and high average systolic blood pressure during radial artery compression were the risk factors of operative palmar swelling. However,multivariate stratified analysis of risk factors showed that systolic blood pressure ≥130 mmHg during radial artery compression increased the risk of ipsilateral palm swelling in patients with diabetic history (OR=1.09,P<0.001). In patients with or without heart failure signs and hypertension history,when the systolic blood pressure during radial artery compression was more than 130 mmHg,the 2 groups increased the risk of palmar swelling on the operative side. Furthermore,trend regression analysis showed that the average systolic blood pressure during radial artery compression was between 130 and 149 mmHg. Every 10 mmHg increase in systolic blood pressure had increased the risk of palmar swelling by 3.52 times[95%CI (1.37,13.22) ]. Conclusion During the period of radial artery compression,the average systolic blood pressure of patients is 130-149 mmHg,and the risk of palmar swelling on the operative side is the highest. It is suggested that clinical nurses should comprehensively consider the changes of systolic blood pressure when formulating the prevention plan for complications of palmar swelling on the operative side.

Investigation on the quality of online teaching of nursing teachers in undergraduate education

Objective To investigte the work status of nursing teachers in colleges and universities under the background of online education,including problems and difficulties. Methods A self-designed questionnaire “national survey on online teaching quality of undergraduate nursing education(teacher version)” was used to conduct among 1 223 nursing teachers and clinical teachers from June 10 to June 30,2020. Results Among the investigated teachers,783 teachers(64.0%) had no experience of online teaching before February 2020,but 1 114 teachers(83.8%) accepted online teaching. From February to June 2020,teachers could provide students with all-round learning support and rich teaching activities. Compared with the theory teaching,259(57.9%) and 109(44.7%) teachers thought that the effect of experimental teaching and clinical practice teaching was worse. Network failure (72.4%),uncontrollable online classroom state(60.8%) and inactive interaction of students(56.6%)were the main difficulties of online teaching. The overall level of teachers’ self-evaluation of online teaching quality was satisfied,and the score of teaching preparation was the highest,with an average score of 4.1±0.7. Conclusion Most teachers lack online teaching experience,but they had a positive attitude towards online education. The effects of online experiment teaching and practice teaching were not ideal. The quality of self-evaluation teaching was satisfactory. It is suggested that nursing teachers should provide better nursing education through the combination of online and offline education in the future.

Analysis of health status and its influencing factors of community residents based on the intelligent health monitoring system

Objective To investigate the health status of community residents and to analyze related influencing factors. Methods A descriptive cross-sectional study was conducted to obtain data of 4 311 community residents in China from April to May,2019. Intelligent health monitoring system,including their general characteristics,health behavior,health status,results of generalized anxiety disorder scale-7(GAD-7) and patient health questionnair-9(PHQ-9),was used as the suvery tool. Descriptive statistics,chi square test and logistic regression were used for data analysis. Results The average age of the participants was(29.25±11.88) years old,and the prevalence of chronic diseases was 13.8%. There were 1 480(34.3%) patients with anxiety and 1 620(37.6%) with depression. The smoking rate and drinking rate were 12.2% and 32.3% respectively. Age,education level,being married,meal regularity,additional meal in the morning,eating dinner,meat-based diet,salty diet,other special diet,drinking alcohol,taking vigorous exercise,anxiety and depression were the influencing factors of chronic diseases. Conclusion The health condition of community residents was generally good,while the symptoms of anxiety and depression were common,which implies that much attention should be paid on the emotional health. The risk factors of chronic diseases were old age,being married,irregular meal,meat-based diet,salty diet,other special diet,drinking alcohol,no vigorious exercise,depression and anxiety. These findings can provide suggestions for the community to develop interventions to promote residents’ health.

The establishment and practice of postoperative hypothermia rewarming program for severe burn patients

Objective This article summarized experiences of postoperative hypothermia management for severe burn patients. Methods We established multidisciplinary management team and formulated postoperative hypothermia rewarming program for severe burn patients based on evidence,including set-up of rewarming unit,clarification of starting and ending criteria of rewarming,the method and frequency of core temperature monitoring,complex rewarming interventions for burn patients,management of rewarming equipment,training and evaluation of nurses' rewarming skills. Results The continuation time of hypothermia was decreased from(274.38±149.43) min to(107.89±65.39) min. The compliance of complex rewarming interventions increased from 21.88% to 83.33%. The accuracy rate of core temperature monitoring was improved from 18.75% to 97.22%. Conclusion The postoperative hypothermia rewarming program of severe burn patients is beneficial to improve the quality of nursing care.
